To enrol your child for a nursery place, you will initially be asked to complete a booking form. This provides us with your personal informtion and the sessions you require. 

Once your sessions have been agreed and you have recieved your government funding letter (if required) you will be asked to complete our enrolment forms. In this pack there is:

• A personal information form 
  This asks for personal details, GP details, Medical, Allergies and any professional involment.
   
• Alternative contact form 
  This is incase of an emergency and we cannot reach the main parent/carer and details of any other person who may collect.
   
• Consent form
  This is for you to give us permission to apply suncream or medication in case of emergencies.
   
• Image consent form 
  This is for you to give us permission to use your childs image on displays, learning journeys, paper, website and our facebook page.
   
• Outings consent form 
  This is for you to give us permission to take your child out of the builing on local walks or trips.
   
• Sickness policy 
  This is for you to be aware of our exclusion periods for particular illnesses.
   
• Payment policy 
  This is for you to sign to say you agree to all the terms and conditions around nursery fees and payment.
